Home Based Primary Care

Home Based Primary Care (HBPC) is a healthcare team that visits you in your home based on your individual need.

It is designed to serve

 - Chronic diseases that are difficult to manage such as:

  • Heart Disease
  • Parkinson's Disease
  • ALS
  • Dementia
  • COPD
  • Diabetes

 - Other Chronic Conditions

 - Very disabled Veterans

 - Veterans with a history of ER visits and hospital stays.

Services Not Provided by HBPC

  • Emergency Care
  • Daily or on-call nursing
  • Medication Delivery
  • Homemaking Services
  • VA Specialty Care Services

Goals of HBPC

  • To provide the best health care possible to the Veteran within his or her home.
  • To maximize the Veteran's independence
  • To support the Veteran's quality of life
  • To provide education and support to family & caregivers
  • to demonstrate appreciation to our Veterans through respectful and proffessional service.  

How to Enroll

Talk to your VA Primary Care Provider to have a consult place dor a referral to HBPC.
